At Clarinda Regional Health Center we offer an online records request service that verifies your identity by asking for a photo of your driver’s license, which you can take via webcam or smart phone. There is no additional charge to use this service. Please use Chrome, Firefox or Safari for your web browser.
To have a copy of your medical records sent to another medical facility, please request to have your records released to a specific provider outside our organization by using the online form below or alternatively, you can print and mail or fax this form to make a records request. Send the completed paper Authorization form to:

For Medical Providers
At Clarinda Regional Health Center we offer an online records request that allows for your medical providers to access your medical records for continuity of care. You must be a medical provider to use this specific online records request feature. There is no additional charge to use this service. Please use Chrome, Firefox or Safari for your web browser.

You can use the button provided above labeled "Request Your Medical Records Online" which will take you to our new platform that makes filling out the necessary information easier than ever! Your records will be provided to you via email or mail through this option.
You can also access many of your medical records online by signing up for our Patient Portal account through MyCareCorner. Learn more about that option here.
1. Your doctor’s office can contact us directly to obtain your medical records by calling 712-542-8302.
2. Your doctor’s office can be faxed records that you personally request. You must still submit the appropriate authorization form, making sure to include the physician’s name, mailing address, phone number, and fax number.
Please note that a new form must be completed for each physician to whom you need your medical records mailed or faxed.
3. Lastly, a medical provider can also request your records themselves for continuity of care reasons by going to the “For Medical Providers” section on the website and clicking the button to request a patient’s medical records.
If you need another individual to pick up the copy of your records, you should record that person’s name in the authorization. That person may then pick up any copies AFTER producing a valid picture ID so that we can verify their identity.
It varies but is typically processed within five (5) business days. If you have already filled out the necessary information and still do not have your records, please contact the HIM office by calling 712-542-8302.
There are several ways to get your medical records. First, in the questionnaire above, let us know how you want to receive them. By using the link above you can have them emailed or mailed.
If you stop by in person and opt to fill out the paperwork instead of the online questionnaire above, we may be able to provide you with a few records within minutes. An example might be lab draw results or records from a specific doctor's visit. For larger requests or to release your entire medical history you will need to use the questionnaire above and will receive your records via your preferred method within five (5) business days.
There is no cost for a patient to obtain their own medical records.
