Welcome to Medicare Visit

  • One-time preventative visit for people in their first year of Part B
  • Scheduled with your medical provider – takes about 30 minutes
  • Not a full physical exam – focuses on preventative care
  • Develop plan for keeping you up-to-date on Medicare covered services
  • No cost to patient

Annual Wellness Visit

  • Yearly visit 1 year after your Welcome to Medicare Visit (or 1 year after your first year of Part B if you did not have a Welcome to Medicare Visit)
  • Scheduled with a Registered Nurse – takes about 30 minutes
  • Update preventative services and any changes to health history
  • No cost to patient

Chronic Care Management

  • For Medicare patients who have two or more chronic conditions
  • Typically, a monthly phone call (or more if needed) from a nurse, but can include face-to-face visits
  • Personalized care plan for management of chronic conditions
  • Copay does apply

Senior Health Insurance Program (SHIIP)

The Senior Health Insurance Information Program (SHIIP) is a free and confidential service offered through the state of Iowa to help people find answers to important questions regarding Medicare and other health insurance options.
